Overview

The Home Infusion and Specialty Pharmacist performs, coordinates, and supervises pharmacy services. This includes dispensing drugs/supplies, preparing sterile products, and ensuring safe, effective therapies in home and alternative settings. You will also conduct clinical assessments, patient education, care planning, and therapy monitoring.
Key Responsibilities
  • Pharmacy Operations: Prepare and dispense drugs, supplies, and equipment.
  • Sterile Compounding: Create sterile products for safe home and alternative-site therapies.
  • Patient Care: Lead clinical assessments, patient education, care planning, and outcome monitoring.
  • Leadership & PIC Coverage: Assume manager/Pharmacist-in-Charge (PIC) duties in their absence to maintain workflow and patient care (excluding tasks restricted by law).
Requirements & Logistics
  • Schedule: Hours vary and include on-call rotations based on census and program needs.
  • Primary Software: CareTend, NewLeaf, EPIC, TherigySTM, and Abacus.

Responsibilities

Clinical Patient Care & Monitoring
  • Therapeutic Monitoring: Conducts initial and ongoing assessments of patient, drug, and disease information to identify real and potential drug-related problems.
  • Care Planning: Formulates prioritized, goal-directed plans of care based on determined patient problems and desired clinical outcomes.
  • Intervention: Intervenes as appropriate to improve prescribing, administration, and monitoring practices facility-wide.
  • Consultation: Provides drug information and consultations to patients, families, physicians, and healthcare professionals.
  • Infusion Therapy: Programs infusion control devices and educates patients, caregivers, and staff on their safe operation.
Pharmacy Operations & Dispensing
  • Medication Preparation: Supervises, coordinates, and participates in the preparation of sterile admixtures.
  • Dispensing: Dispenses all pharmaceutical and related supplies/equipment strictly in accordance with federal, state, and local laws and standards of practice.
  • Leadership: Supervises pharmacy operations and directly oversees support personnel.
Compliance & Administration
  • Documentation: Completes documentation per policy to maintain thorough and accurate clinical records.
  • Regulatory Maintenance: Maintains rigorous pharmacy records in accordance with all applicable laws and regulations.
  • Billing & Authorization: Performs billing functions (manual computer entry, billing form generation), identifies payors, and obtains authorizations prior to delivering medications or supplies.
  • Performance Improvement (PI): Supervises, collects, and analyzes PI activities specific to pharmacy functions.

Qualifications

EDUCATION
  • Bachelor of Science in Pharmacy | Required Degree Completed
CERTIFICATIONS & LICENSES
  • Licensed Pharmacist – Illinois Department of Financial and Professional Regulation (IDFPR)

Salary Range

The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
