Overview
Responsibilities
Serve unpaid caregivers of persons with dementia through the Star C evidence based intervention.
Manage a support group for caregivers of persons with dementia that meets at least monthly.
Conduct education opportunities for clients about dementia, brain health and if needed Stressbusters for caregivers of persons with dementia, aging mastery, Fit & Strong, BingoCize.
Conduct ongoing outreach to clients at least monthly.
Assist with implementation of Memory Café events for persons with dementia and their family caregivers.
Work with clients in home visits and through screen-based technology.
Collect needed data from clients and capture case notes for intervention. Analyze data to look for trends.
