Overview
Responsibilities
Documents assessments on assigned patients, given that an RN has charted the initial assessment
Charts on care plans, which the RN has opened; will have a RN open any new care plans
Communicates with providers to receive orders and coordinate care for patient
Completes dressing changes and wound care
Completes trach care
Inserts NG tubes and maintains feedings through NG tube
Cares for and changes ostomies
Reinforces education and discharge instructions
Qualifications
Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
Education:
College Diploma: Nursing (Required)
