Position Summary
$5,000 sign on bonus available!
Person in this position functions on a multidisciplinary team to provide care to persons with serious and persistent mental illness in community based settings.
Qualifications
• Licensed Illinois nurse. RN Needed.
• Experience in psychiatric nursing or with mental health population preferred
• Knowledge of psychotropic medications
• Excellent written and oral communication skills,
• Ability to quickly establish rapport,
• Computer proficiency required,
• Ability to navigate and complete documentation in the electronic healthcare record,
• Ability to accurately complete required documentation within the prescribed time frames
• Ability to present and coordinate lectures for clients, parents, staff or community resources,
• CPR training or complete within three months
• Valid driver’s license and ability to meet Agency insurance requirements (designated programs),
• Must have own transportation (designated programs).

Responsibilities
1. Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
2. Demonstrates essential nursing skills; respond to medical crises when on site
3. Assesses physical/medical needs of assigned program clients,
4. Coordinates medical appointments for assigned program clients. Provide transportation and support
to clients for medical appointments when needed.
5. Completes Medication Administration and Training for program clients
6. Completes prior authorizations as needed for clients medications
7. Maintains communication with team leader and coordinator on medication assistance requests and budgets
8. Builds relationships with medical community and providers and advocates for program clients to assure they obtain appropriate care
9. Provides education to program clients regarding medical problems and prevention issues
10. Provides education on nutrition, wellness, medications and symptom management to program clients either group or individual sessions
11. Completes required Rule 132 documents including progress notes, MHA’s, tx plans and reviews
12. Be familiar with court proceedings and available to make court appearances on behalf of Rosecrance clients.
13. Serve as a member of the team and participate in all team meetings and activities.
14. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
15. Deliver exceptional customer service consistently to every customer.
16. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance
17. Assume other related responsibilities as delegated and assigned by management.
Schedule
- Hours: Monday - Thursday 8:30am - 7:00pm (40 hours per week).
- Shift: 1st shift
Work Location
Rosecrance Walnut, Champaign, IL.
- Work Mode: Onsite (Community Based)
Compensation & Rewards
- Starting Pay (based on education, experience, and credentials)
- RN - $31.00 per hour
- BSN - $32.50 per hour
