CHAMPAIGN COUNTY OR PIATT COUNTY (please indicate in your cover letter/application which county you’re applying for)Seniors thrive when they remain in their own homes as long as possible. We’re seeking a highly motivated, compassionate, organized person to work directly with seniors in Champaign, Piatt, and Douglas Counties to help them avoid assisted living or skilled nursing facilities for as long as possible.A KEEN EYEConduct assessments of older adults; catch details and read between the lines to address root problems/barriers. Monitor seniors’ emotional, mental, and physical well-being; be ready to intervene as needed.HIGH ENERGY & INDEPENDENTWork in the community (residences, medical facilities) and educate other professionals about aging issues. Manage your own schedule and caseload without constant supervision.WILLING TO BE A SQUEAKY WHEELAdvocate for seniors, connecting them to needed services (nutrition, housing, legal, medical/pharmaceutical). Act with urgency when needed.COMPASSIONATE & EMPATHETICSupport seniors through life changes, grief, and family difficulties; be sensitive to cultural/socioeconomic differences. Help ensure no senior falls through the cracks.WILLING TO TRAVEL (LOCALLY)Home visits throughout Champaign or Piatt County; attend community events. Family Service offers services in three counties; occasional visits to the other counties. Mileage reimbursed at the federal rate.A NATURAL-BORN SCRIBEKeep timely, observational case notes; ensure forms are correct and follow-up tasks scheduled. Maintain internal documentation (mileage, daily schedule). Strong written and verbal communication.BOTH A TEACHER & STUDENTPresent at meetings/forums on senior issues and available services; help others connect with Family Service. Bachelor’s degree preferred; able to obtain certifications (e.g., SHIP, I-AIRS, SAGE Care). Attend advanced clinical training.TEAM PLAYERFast-paced work, database use, confidentiality, and seniors-first mindset.OTHER DETAILSLooking for at least 1 year of case management experience.
