Resident Services Social Worker – Skilled Care Admissions Coordinator
ClarkLindsey, a premier Senior Life Plan Community in Urbana, IL, is seeking a compassionate, licensed, detail-oriented Social Worker. This dual-role position is essential in managing the admissions process for both short-term rehabilitation and long-term skilled nursing care residents at MeadowBrook and the Green House Homes, while also providing psychosocial support, care planning, and mental health services across all levels of care.
The ideal candidate will be a proactive, empathetic, licensed professional with experience in skilled care admissions, insurance companies, MDS assessments, discharge planning, and mental health counseling, and committed to fostering a holistic and person-centered approach to resident wellness. The schedule is 9am – 5:30pm Monday – Friday, with one weekend on-call shift per month.
Key Responsibilities:
Admissions & Care Planning
· Act as the main contact for prospective residents and their families.
· Handle referrals, pre-admission assessments, and the entire admissions process.
· Work with nursing, therapy, and social services to create and update care plans.
· Join team meetings to review resident progress and plan discharges.
· Manage insurance notifications, appeals, and communicate with all parties involved.
· Complete and submit MDS assessments following regulations.
Grievance Officer Designee
- Support the grievance process by receiving, documenting, and tracking a variety of resident concerns, including care issues, missing items, food complaints, equipment problems, and interpersonal conflicts.
- Assist in investigations, ensuring confidentiality.
- Respond to residents and ensure all grievances are addressed promptly and fairly.
Resident & Family Council Coordination
· Facilitate monthly Resident Council meetings including documentation of minutes and coordinating responses to resident concerns.
· Organize quarterly Family Council meetings, document minutes, and serve as a liaison to coordinate responses to family concerns .
Psychosocial & Mental Health Support
· Conduct mental health assessments and provide short-term counseling to residents.
· Refer residents to external providers for long-term mental health support as needed.
· Lead support groups and educational sessions for residents and families.
Resident Services Team Collaboration
· Provide cross-functional support to Resident Services roles across campus
· Guide residents and families through levels of care and support smooth transitions between services.
· Actively participate in quality assurance initiatives and compliance audits to support continuous improvement.
Qualifications:
· Master's degree in social work (MSW) and current LSW or LCSW licensure in Illinois.
· Minimum 2 years of experience in a senior living, healthcare, or skilled nursing setting.
· Proficiency in MDS 3.0 and knowledge of Medicare regulations.
· Strong interpersonal, organizational, and communication skills are important.
· Demonstrated experience in care planning, insurance companies, discharge coordination, and mental health support.
Preferred Skills:
· Knowledge of state and federal regulations for skilled care admissions.
· Ability to work collaboratively in a fast-paced, interdisciplinary environment.
Compensation and Benefits
The pay range for this position is $62,000 - $65,000, salary, and is based on experience. Bonuses equaling up to 2% of salary annually are available based on performance, generally in the spring and fall each year.
