Overview
General Summary of Duties
Christie Clinic's department of Physical Therapy is seeking a full-time Physical Therapist at the Fields clinic from Monday-Friday with varied hours from 7:00am-6:00pm, with no nights or weekend requirements.
Duties include evaluating, planning, interpreting and implementing specific physical therapy treatment programs for patients.
Job Qualifications and Expectations
JOB DUTIES: (This list may not include all of the duties assigned.)

Interact and practice in collaboration with a variety of health professionals.

Consult, educate and engage in critical inquiry. Clarify referring provider’s instructions and recommendations as needed.

Direct and supervise the Physical Therapy services, including support personal.

Examine, evaluate and establish Physical Therapy diagnosis and prognosis.

Establish plan of care and treatment program in coordination with support staff and medical staff.

Facilitate coordination and communication of relevant patient information with pertinent members of the medical team and the patient.

Document patient information into the chart per department and APTA guidelines in a timely fashion.

Maintain patients’ records, and charts per insurance guidelines.

Instruct patients, families, caregivers and communities in activities to promote and optimize the physical therapy services.

Select, apply and modify procedural interventions based on the patient evaluation.

Evaluate charts and reports on patient's progress. Review and co-sign Physical Therapist Assistant notes in timely fashion.

Select most appropriate billing codes. Demonstrate understanding of insurance rules/regulations and billing practices.

Recommend physical therapy policies and procedures concerning services provided. Maintain quality assurance program, safety, environmental and infection control.

Recommend solutions to patient care and department problems.

Conduct and participates in clinic educational programs.

Assist with data collection and prepare reports. Maintain patient statistics.

Maintain unit in a clean and orderly manner. Keep equipment in good working condition.

Perform related work as required.
REQUIRED QUALIFICATIONS:

Minimum of Bachelor’s Degree in Physical Therapy

Graduation from accredited Physical Therapy Program
PREFERRED QUALIFICATIONS:
-
Doctorate in Physical Therapy
CERTIFICATE/LICENSE:
-
License to practice physical therapy in state of Illinois
-
CPR Certification
TYPICAL PHYSICAL DEMANDS:
Demands include sitting, standing, walking, bending, stooping, stretching and lifting up to 75 pounds. Other demands include the ability to use department equipment, the ability to facilitate prescribed treatment, including patient handling and the ability to communicate nature of treatment to patient and prepare provider's report. Hearing within, or correctable to, normal range, vision correctable to 20/20 and manual dexterity for the operation of office equipment is required.
TYPICAL WORKING CONDITIONS:
Work is performed in a clinical setting with possible exposure to diseases, toxic substances, radiation, medicinal preparations and other conditions common in a clinic environment. Requirements may include working irregular hours.
PAY AND BENEFITS:
The estimated pay range for this position is exclusive of fringe benefits and potential bonuses. Final offers are based on various factors, including skill set, experience, qualifications, and other job-related criteria.
