Overview
Responsibilities
EDUCATION: Develops and maintains comprehensive educational materials relative to the continuum of cancer care; Provide educational materials and resources to patients hospitalized with a survival procedure; Provides educational material and resources to patients within the comprehensive cancer center setting; Initiates, organizes, and evaluates education programs; Uses evidence based practice to promote patient / family / caregiver engagement, activation and empowerment; Community Education; Participates in community outreach programs.
PROGRAM COORDINATION: Program Development; Collaborates with the provider group and administrative leadership to develop practice guidelines, including processes of care, care protocols, policies and procedures; Develops and maintains division-specific documentation tools in collaboration with IT resources; Organizes and oversees the multidisciplinary clinic; Collaborates with leadership to identify and report key operational and programmatic metrics; Communication; Demonstrates positive communication skills with patients, families and co-workers; Promotes positive communication in peer interactions; Communicates effectively with the interprofessional team to effectively and efficiently provide high quality patient care; Research; Coordinates with research staff as indicated through the treatment plan; Promotes the development and delivery of evidence-based practice.
Coordinates patient care activities and transitions of care for patients with the interdisciplinary teams Coordinates follow up appointments throughout treatment and survivorship Facilitates appropriate follow up of diagnostic results Initiates and updates individualized patient centered plans of care Participates in the development and delivery of patient care summary and survivorship plans.
