Overview
The registered dietitian provides medical nutrition therapy to ambulatory patients individually or in a group setting. The registered dietitian provides a nutrition assessment, determines and implements a nutrition care plan and intervention. Develops and implements nutrition education programs for the general public, community organizations and for medical and clinic staff.
****This position requires travel to Rantoul one day a week.
Responsibilities
Essential Functions |
| Provide nutrition services and medical nutrition therapy integrated with the patient's medical goals. Identify and provide age specific nutrition counseling to meet the cultural needs of the patients and their families
|
| Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines.
|
| Develop, review, update and implement educational materials to meet the needs of patients and professionals
|

Completes clinical and educational assessment on all patients. |
Develops individualized teaching plans for self-management (Diet, Medication, Self-care) |
Implements evidence based clinical practice guidelines for all area of nutrition education |
Provides education to all patients utilizing insulin pump. |
Collaborates will all members of the health care team |

Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR) | And |

Certified Diabetes Care and Education Specialist preferred |

Achieve and maintain qualifications for level as outlined in the ladder pay practice. |
