Job Description

JOB SUMMARY:
The Cancer Navigator acts as an advocate to assist patients in navigating throughout internal and external systems in order to facilitate the diagnosis and treatment process. Applies the nursing process through assessment, planning, implementation and evaluation of the patient in order to provide the optimal level of patient care. Duties include clinical practice, education and program coordination.

EDUCATIONAL REQUIREMENTS
Diploma (Not HS) in Nursing

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois upon hire and National Nursing Certification within 3 years.

EXPERIENCE REQUIREMENTS
Two (2) years Nursing and. Experience in Oncology

SKILLS AND KNOWLEDGE
Must have good leadership/professional skills as well as strong communication skills. Role modeling through the prioritization of activities and the setting and attaining of professional, personal, and organizational goals. Demonstrates efficient management of time and resources. Demonstrates the ability to assess, problem-solve, and critically evaluate unique circumstances to make evidence based decisions. Coordinates and attends appropriate organizational and departmental meetings where employment related matters, policies, procedures, and programs are discussed. Maintains leadership role as appropriate within division meetings. Actively participates in professional committees. Maintains membership in national and/or local professional organizations.

ESSENTIAL FUNCTIONS:
  • Independently applies the nursing model to provide direct care to patients; Plans, administers, and evaluates patient care in practice; Coordinates patient care activities and transitions of care for patients with the interdisciplinary team through the longitudinal care processes of cancer treatment (including processes of treatment, scheduling, internal/external referral, and follow up); Coordinates follow up appointments through the treatment and survivorship surveillance continuum; Facilitates appropriate follow up of diagnostic results; Initiates and updates individualized patient centered plans of care; Documents the nursing plan of care and associated care coordination and transition management in the medical record; Delivers holistic patient care in the form of direct care, psychosocial support, and age-appropriate education; Participates in the development and delivery of patient care summary and survivorship plans; Promotes the ethical care delivery through professional nursing practice including patient advocacy, coaching and counseling.
  • Develops and maintains comprehensive educational materials relative to the continuum of cancer care; Provide educational materials and resources to patients hospitalized with a survival procedure; Provides educational material and resources to patients within the comprehensive cancer center setting; Initiates, organizes, and evaluates education programs; Uses evidence based practice to promote patient / family / caregiver engagement, activation and empowerment; Community Education; Participates in community outreach programs.
  • Collaborates with the provider group and administrative leadership to develop practice guidelines, including processes of care, care protocols, policies and procedures; Develops and maintains division-specific documentation tools in collaboration with IT resources; Organizes and oversees the multidisciplinary clinic; Collaborates with leadership to identify and report key operational and programmatic metrics; Communication; Demonstrates positive communication skills with patients, families and co-workers; Promotes positive communication in peer interactions; Communicates effectively with the interprofessional team to effectively and efficiently provide high quality patient care; Research; Coordinates with research staff as indicated through the treatment plan; Promotes the development and delivery of evidence-based practice.
