Job Description

JOB SUMMARY:
The Clinician/Admissions role is responsible for determining the eligibility and preparation of patient/families for hospice care. This can be done in hospitals, assisted living or long term care facilities, and/or the home environment throughout our service area. This position is responsible for all activities designated to enhance and coordinate relationships with referral sources, increase referrals, and assist in timely admissions.

EDUCATIONAL REQUIREMENTS
Diploma (Not HS) in Nursing

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois upon hire and Driver's License Illinois upon hire and Proof of Auto Insurance upon hire.

EXPERIENCE REQUIREMENTS
Hospice experience

ESSENTIAL FUNCTIONS:
  • Demonstrate understanding of Hospice Regulatory requirements/benefits including covered services, recertifications, transfers, and non-certifications.
  • Consistently completes all admission documentation within policy guidelines. Returns admission documents to office complete and in timely fashion within policy guidelines.
  • Comprehends department specific indicators.
  • Performs case management activities of assessment, coordination, planning, monitoring, implementation, and evaluation. Interacts with patients, caregivers and families to assess, plan care, arrange services, monitor, and provide support and education.
  • Completes required admission/recertification corrections within 24 hours of receiving them.
  • Completes visit notes and telephone conversation notes and transmitted timely according to policy
  • Ensures physician orders are written timely, corrections to care plans are entered and transmitted according to policy time line.
  • Reviews verbal orders with MD office, written, and completed immediately and transmitted within time line established by policy.
  • Ensures patient visit strings are added to the record to make sure visits are not missed.
  • Assess patients for appropriate placement in Hospice per Medicare guidelines.
  • Make home visits to monitor for signs/symptoms associated with end stage disease and end of life.
  • Implement interventions to palliate symptoms in concert with attending physicians.
  • Make home visits to assess and pronounce death, do post-mortem care, notify County Coroner of patient passing, notify Funeral Home and physician(s).
  • Provide comfort for the family and make appropriate referral to Bereavement Coordinator.
